Svetlana Siber Isamar is wonderful and very professional. She did a dip mani and pedi. The manager Alicia doesnt book every minute. Its very peaceful and clean. Ciynthia offers coffee, drinks, and water.

Anthony C This place is fantastic from A-Z. From Patty running the front desk, Layla who’s washing hair, to Francesca always being pleasantly present…the whole crew there are great. This spot is so good I might even go even if they weren’t great at doing hair. With that said they are fantastic, I personally work with Sam who is a cut above from anyone else I’ve ever worked with. This place is highly recommended.

Abby Robinson Traci was so sweet and professional. She was so knowledgeable about the products she used and was very helpful in answering any questions I had! I highly recommend getting your hair done by her because her service was to die for! Best massage ive ever had! You can tell shes passionate about doing hair just by hands and her sparkling personality! 10/10 Will be back just for her!

Marie M. I love coming here on a monthly basis. Oftentimes, we make it a Girls Day Out date with one of my daughters, sister or a niece. I’ve had the greatest experiences with both Luz Maria and Maya who I can request online or by phone. The hostess is always super pleasant and attentive too, offering drinks from their regular drink list. I do wish they added sparkling water, but in its stead, I order a mimosa. When it comes to a style, whether I choose the Mai Tai or a regular blow dry, my time here feels exclusive, regardless of how many other customers are also getting pampered. The ambiance is relaxed but somehow fun as well. The decor, with the elegant framed mirrors, is perfect and I just can’t recommend them enough!

Grace Redden I had an amazing experience with Izzy! She is the sweetest and incredibly knowledgeable. She not only gave me a great trim but also took the time to explain how to style my hair at home and which products she uses. I hadn’t touched my hair in two years because I was hesitant to trust anyone, but after hearing so many great things about her and FORM, I decided to give it a try. I’m so glad I did—I absolutely love my hair and have finally found a stylist I can trust. If you have curly or textured hair, you definitely need to see Izzy!

Anastasiya is a wizard with color—she suggested the Opium Airtouch technique and it was a game-changer! Ive never been happier with my hair: stunning color thats so natural-looking and these beachy waves that made me feel like I just stepped off a magazine cover. Plus, they use the best products, and it really shows in the health and shine of my hair. Im already planning my next trip from Dallas just to have Anastasia work her magic again. Ami De La Soul Ashley did an amazing job with my hair. Not only does she ask you questions to make sure your vision is a possibility but she will always work at your hair nonstop until she gets the look exactly how you’d hope for but better. She works hard, shows great heart, and I have been consistently going to her for my hair needs since last year.
